Causes of volume and shape loss

We should first talk about the anatomy of the lips, which will then allow us to better understand what changes in this anatomy occur over time. 

  • Layer 1: The lips are made of a pink skin called vermillion, which is the smooth part of your lips where you put lipstick. The change from pink to regular skin is called the vermillion border. 

  • Layer 2: The second layer of the lips is made primarily of fat and salivary glands. This layer is the most important factor in determining the plumpness of your lips, and how much pout you have. 

  • Layer 3: The most inside layer is the muscle called the orbicularis oris. This muscle is what moves the lips, and allows you to make a puckering movement or a curling in movement. This muscle actually adds the most volume to the lips. 

Each of these layer’s changes over time, and in advanced age, each layer develops certain characteristics. 

Vermillion Skin (Pink Part Of Lip)

Loss of elastin in the lips is similar to loss of elastin in skin for the rest of the body, which causes this layer to thin. Also, because of many years of the underlying muscle movement, small wrinkle lines start to develop. These are often unflatteringly called “Smokers Lines”, even if the woman doesn’t smoke! Lip filler often greatly improves these lines by bringing in much needed hydration. 

Deep Lip

The deeper layer, composed of fat as salivary glands, typically undergoes fat atrophy over time. This loss of fat is most notable observed in the lip plumpness-think of a plump baby’s face and lips, which has new fat! This is an important consideration in the aging lip, and the most obvious area where lip filler can be of benefit. 

Orbicularis Oris

The orbicularis oris muscle adds the most volume to the lip. As time goes, every muscle in the body will slowly lose volume, including the lip muscle. Because this muscle is moving the lips, and not lifting objects (like the biceps), it does not lose function in advanced age, only volume. As this muscle shrinks, it can cause a curling in of the lip, which a thicker lip filler such as Restylane Kysse can greatly improve!
